This may be of interest to some.

We live in Spain, I am retired. My wife, born 1971, automatically got Spanish National Health cover when she came here. The UK actually pay about £2,500 a year to the Spanish health service for each entitled person.

I have recently been looking at how I can protect my wife ‘generally’ when I die. I checked with DWP and was assured she will continue to be entitled to free health cover in Spain. Those living in UK of course get the same protection

If you are talking about in the UK then it's on arrival. Just register with a doctor as soon as possible.
