flr...pls pls pls help!

I've been reading post in "search function" about flr but still struggling about the following questions:

1. page 3-Applicants full name (It doesnt say which comes first) Do I need to write first name then surname?
2.page 4- 1.6 "Any other name(s) by which you are or have been known...(I read this question posted by other member but still want to make sure,can I write it my middle initial?coz in 1.5 says " surname or family name and 1.4 says "your full name as in your passport"....

I did not ammend my passport yet and want to apply flr by post.Pls help...

Hello Pat,

full name ....your name (they often refer here as Forename) followed by your surname example : Sandy Prince (it depends if you want to put middle initial then it will be Sandy A. Prince)

other names ..dont put one if you dont have any

your full name as in your passport ..Sandy Lim Aquino ..
