sss maternity benefit

Hi... hope anyone can give me an advice.
According to the Philippine Embassy, i can claim a maternity benefit from my sss, they gave me several forms and then one of the requirement is, since im ceasarean they wanted me to get an operating room record and certified by the hospital where i gave birth.Does anyone has an idea about it??? Thank you.

yes ,If you are an SSS ''paying'' member you are entitled to avail the maternity benefits provided you meet the requirements...please check this links
