The FLR application form ask for:-
Your full name as in your passport or travel document
I don't know when you are intending to apply for FLR, but it's always a good idea to review these forms well in advance. At least you will know exactly what is required and have sufficient time to get all the supporting documents in order.
Good Luck:xxgrinning--00xx3:

hi,
or you can go to the philippine embassy in london and apply for a name change in your passport, the forms are on their website, all they do is stamp the second page with the name change, costs about £15.00, fill out the forms, they must be witnessed by two people and provide passport size photos and marriage certificate, if i remember right it took a couple of days or was it the same day? and we then used her married name for her FLR.
oh btw, when traveling allways take the marriage certificate with you to check-in as they see the name change and usually as to see the original M/C.

HI MSBelinda.
Wehave just made a successful application for my wifes ILR. Like you she also has her Filipino passport in her maiden name and not her married name.
You MUST use the exact name that is in your Fil passport on the FLR application and anything else to do with the home office.
I have not looked at the FLR application form but on the ILR form there is a section below the applicants name that says "any other name or names that you are known by"
In this section my wife wrote her married name. Our application was checked by the local authority visa checking service and they said that this is the correct thing to do

That was my question too during I fill up the form, I used my not married name but I think they accepted it because Im done with the biometrics stuff and until now Im still waiting for the result. Dont worry if you use your single name in the form it is understood that you are married because you attached marriage certificate plus the signature of your husband, so they are not that stupid to understand the application. Did you send your application already?
