confusion here

am currently on writing the data on the ILR application my question is..

a) is this the correct format am using..http://www.ukba.homeoffice.gov.uk/sitecontent/applicationforms/settlement/formsetm0420091.pdf
b) in the page SET(M) Application form (Version 02/2012) - page 10(question 8-9-10)
im confuse what to put..?? is it the date where i apply the spouse visa..as i rem that is a biometric?? do i need to put the date..
and if so could anybody pls give mt the address of the VFS /Manila.. who issued the spouse visa:)
though i need to include to copy yhe guidance note..? when i submit the application..

Look here at the top right-hand (http://www.ukba.homeoffice.gov.uk/visas-immigration/settlement/applicationtypes) for all forms and information

Yes, just put the info from when you applied spouse visa (date place etc)

It wasn't the VFS that issued the visa.
They merely acted as the agent.
The visa was issued by The British Embassy, Manila .... which is what you need to write on the form

The question in the form is asking where her fingerprints/biometrics were taken and not who issued her visa..and it was at VFS..so she should write there the VFS' address..
