I think its asking you to write it down like this 05/12 . Some forms have DD/MM/YY ( 01/05/12 ) or DD/MM/YYYY (01/05/2012) or MM/YY ( 05/12 )

As cold spud said you know the date you were married.

As others have said, it's only you who knows the age of youself and your civil partner when you entered into that contract.
That question does not require any specific date format.

When questions require the answer to be a specific date the nomenclature used is that of:-
DD= day number (eg 21)
MM= month number (01)
YYYY = year (2013) or YY = year (13)
