2. should we put it in a clearbook/envelope/folder
3. should both of us will make a letter
4. may i know the sample of cover letter.

1. There's really no sequence required for the documents. You can follow this https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/270197/sup-docs-settlement.pdf if you want. My advice is to put a table of contents of all the documents you are going to submit. Also put labels on it that the ECO will clearly see. (i.e. what I did with mine in the past, I wrote a label on every category like "English Test Result", "Bank Statements", etc. and attached it with paperclip on them.)
2. When I submitted mine, I put it in a clearbook but the VFS staff collecting my docs asked me to remove it from the clearbook.
3. Hubby and I did write a letter of support or cover letter each for our application.
4. Here is what my hubby stated on his: http://filipinaroses.com/showthread.php/29695-Fianc%C3%A9e-Visa-Application-%E2%80%93-Cover-Letter-Example. With mine, I just wrote a letter containing a brief history of how we met, all my intentions of going to the UK and also our plans as a couple.

My then fiancé only made the letter of support. I did the table of contents though. We used bull clips to make our papers intact.

additional question, if you already completed the online application form, can you make any changes or corrections on it?
