Confusion with Report of Marriage form

these are the part that I am confused about.

1. The header of the form, what should I put here? Foreign service post and date

2. Residence (does it have to be our full address??? if not what can we just put?)

I also have coloured photocopies of our bio page passports, does it hve to be coloured or black and white is ok?

On the website it shows 3 copies, but on the form it is asking for 4 copies.
