Welcome to the forum.

Your mother's full name I think will be her full married name. Her 'maiden name' is her name before she was married to your father.

Hi there,

Basically as I understand it is your mother's full name what she is known by now goes in the first box then the box below is your mother's surname before marriage if that makes sense.

Hi everyone I've just rang up national checking service. They will get back to me in two weeks time to make an appointment. Does anyone know the procedure for national checking service.
