The 3 year period start when you are physically here in the uk. yup, when they stamp your passport on your arrival as i remember regardless if it is a fiancee visa when you get in the uk as long as you are a spouse already when you apply for citizenship and stayed for 3 years and you have your ilr already.
Time here on a visitor visa wouldn't count, but if for example you came here on a student visa, then got married to a British citizen, that would count.

One thing to remember is that when they first process your application you must have been in the UK exactly 3 years ago.
