    I would like to reply about this topic base on my experience...
    I do have a son...his name is...JONATHAN BRIAN SMITH...
    the first name is (JONATHAN) the middle name is (BRIAN) the surname is (SMITH)
    thats what we use when we applied his british passport...but on his philippine passport.
    his middle name is my surname when i was single which is dela cruz....I hope this would help....
