At the embassy in Manila they just do it while you wait and you don't need an appointment. Wherever you choose to go, your intended will have to go with you and she will need to have her cenomar, she can get that from the Philippines NSO (National Statistics Office). I think she may also need her birth certificate.

Something else to be aware of for after your married and she has got a visa is, she will have to attend a guidance seminar at the CFO (Commission on Filipinos Overseas) either in Cebu or Manila. If she doesn't do this and get their sticker in her passport she will not be allowed to board a plane and leave the country even though she has a valid visa. I only mention this last point because this is a very little publicised part of Philippine law and it catches out many Filipina brides and fiancees if they only find out about it a few days before their due to travel or even in some cases, when they are not allowed to board their flight.
