Living in the english speaking world, allows people to infer different meanings for the same awful sounding words, depending upon the situation and the way the insult was delivered.
... WE in the 'west' tend to use the expression somewhat more lightly; it "trips" off the tongue, and - more often than not - we think no more about it.
From OUR point of view, at least, it's certainly not [usually] intended to imply that the mother of the person to whom it is directed is a whore!
