Will there be any chance that you could name some institutions that would sponsor a tier 2 visa to a senior carer.. I got an NVQ level 4, HND, and DipHE qualifications.. I am currently a Tier 4 visa holder..

...Just trying to work out whether I can be classed as an 'institution' or not.
You haven't actually given much information about yourself, so the following response is general and based on being a non-EEA citizen.
Firstly you must already know that the route you plan is not going to be an easy one.
A skilled Senior Care Worker has to be qualified to minimum NQF level 3 and have at least three years relevant experience including 12 months with supervisory responsibilities. Additionally the minimum salary must be £7.80 per hour.
One of the difficulties you face is that most employers are not willing to pay the £7.80 salary.
Secondly non-EEA citizens normally need to apply for positions that are listed on the UKBA Tier 2 Shortage Occupation List here.
If the job is on the shortage occupation list then an employer can offer the job to an overseas person without having first fulfilled the Resident Labour Market Test by advertising the vacancy at least within the UK.
Finally, there are changes ahead for Tier 2 workers.
I strongly suggest you take some time to study the new and full review of the MAC (Migration Advisory Committee) recommended shortage occupation lists for the UK and Scotland which was just published this September 2011

Look also at Annex A as this is a list of organisations and individuals that submitted evidence for the report.
This means that those Healthcare Companies that have UKBA sponsorship certificates
