As Juvy says, this is the so called Checklist. It is the one that was given to my wife for her application. It appears to be a guide only and is not a mandatory requirement, it is the generic checklist used for all circumstances in all countries which is why some does not apply to everyone for example the Tuberculosis bit - not a requirement for a Philippine application.

My wife didnt use this form as such. Nor did she need to. Daft I know. But......

I was told, here it is but don't bother about it. It is a one size fits all guide.
