dfa / nso problem for passport?!

hi guys, ok here's a tricky question to wrestle with!
we are hoping my mother in law can come n visit us but first she needs a passport. problem is her nso birth cert. is in her mothers name, indoy, but she has always used her fathers name, oracoy, on all her id> there is no date of marriage recorded for her parents or it has been lost or destroyed in a fire. the dfa will want to see proof that she is indeed oracoy and not indoy for her middle name it seems. i cant see its such a big deal but apparently it is! anyone come across this type of situation and any advice on how to resolve it so she can obtain her passport? thx, john

I wish there was an easy formula to resolve.
These issues are very very common in the Philippines, but still may take some time and money to fix.
It must be fixed at the local level. Usually the family knows the way (with your special support and contribution). On occasions it may need legal help.
You will need to trust the family.

i was thinking my special support may include a contribution at a local office for someone to correct the birth certificate despite a lack of supporting evidence not that that ever happens, of course.

Or you could follow the very very long and very very bumpy route

i remember my office mate before who had similar problem. she's been using a first name all her life which happened to be somewhat different to what was written on her birth certificate. i think you will be needing some legal help to fix this. It took my office mate at least 6 months or so to correct it and attended court hearings before her birth certificate has been amended.

I had done this years ago before I took my board exam. I have 2 names (rayna joann) but in my NSO birth certificate it only shows "joann" as my first name. We needed to change that to match my credentials for the board exam. I submitted my school credentials, NBI clearance, police clearance, birth certificate from the local civil registry where I was born (showing my complete name) and baptismal certificate to one NSO branch (my uncle knows someone from there that would prioritize my birth certificate amendments since I needed it as soon as possible for the board exam). I'm not sure how much my parents paid for the legal fees but I think P10k more or less (again I'm not sure). Well it took me more than 6 months to get the final documents for the amendments from the NSO branch. Then my father went to NSO main branch in Quezon City to submit the documents for filing and then when I got my NSO Birth Certificate, it now states at the bottom of the original birth certificate the amendments done to it. It's just a sentence stating what my name should be and the reason was a typographic error. I did not attend any court hearing because it's just my first name that needs amendments. I'm not sure about surnames, but I know birth dates, gender and some other errors need court hearings since they need more proof for that.

yes true...it's really a common problem here in the Philippines about the Birth certificate error/missing due to fire or not registered ...they're just (NSO) fixing it now (hopefully)...but of course a lot of trouble to us Filipinos in fixing it not only money are involve but a lot of time and efforts .....in your lola's case Johnny11 ..you need to consult a lawyer about it...good luck

try to get a copy again in NSO then write the fathers surname coz nso follow what they write in the form .....maybe your mother in law have mistake when she fill-up the form....try nyo lang walang mawawala

We started the process to correct a wrong spelling on my wife's birth certificate so the marriage cert and passport were correct for the name she has habitually used but after paying an initial lawyers fee and for the publication in a national paper (cP20k?) we realised that it was going to take longer than the time we had before the wedding so just gave up. Fortunately her given name isnt TOO far away from her actual name, just missing a few letters

She use the name of NSO name Indoy, for easy. only you to do is correct the valid ID. reason DFA not accept if you not submit a original NSO Birth Certificate.

If she wanted name oracoy, to much problem coz you under go step by step processing.

Yes that is a good decision. you do amends the other documents, on what name recorded on the NSO. Johnny! s

No, as far as i know, if a person apply a passport . DFA asked a marriage certificate from NSO if you legal status is marriage. your lola, go to NSO and get a widow certification. if an DFA asked this? If not she get only a NSO birth certificate. i think DFA asked a maximum of 6 valid ID's.

1. Postal ID

2. Senior Citizen ID

3. Voters ID

4. NBI certificate

5. NSO Birthcerticate.

This Requirements above, is good requirements to have a Passport to your Lola. make sure all Valid ID's same name on her NSO birth certificate.

If you cannot comply this requirements because of her 2 valid ID's names problem. i think you hire an attorney to solve your problem.

thx, ive looked at the dfa requirements n it seems the shortest n easiest way is to keep her original nso cert in name of indoy n change the other aceeptable ids to show this name as well. changing the nso is way too long n expensive a route to take
