application form.. need help please!! im going to apply fiance visa soon!! help me..

hello everyone,
im from cagayan de oro city..im going to apply a fiance visa soon,, the ways to apply is confusing me.. i have all my documents already..
and now how to get a fiance visa application form? is there application form that just to print it out then i will
fill up the form by hand written? or i have to do it online??

and how about the payment? where can i pay the fees? im from cagayan de oro city...
do i have to go to manila to pay it?...

please help me guys... i really appreciated it.. thank you in advance..

complete the form online then print it off and sign it.
I think you have to pay it into a designated bank in Manila and provide the payment receipt when you lodge your application and associated documents.
All the info is online.

See this thread. Basically you can pay at any branch of the Union bank. (You cant pay online btw)

"By cash only over the counter at any other branch of the Union Bank. There is a service charge of Php 150.00 per visa application. You will need to bring a photocopy of your passport data page when you pay the fee. The bank receipt will only be valid for 10 working days from the date of payment and you must submit your visa application within this time".

You can fill out the application form online then print it out. Just make sure to be precise and concise with your answer for some questions because the characters are limited.
