confusion here

am currently on writing the data on the ILR application my question is..

a) is this the correct format am using..http://www.ukba.homeoffice.gov.uk/si...etm0420091.pdf
b) in the page SET(M) Application form (Version 02/2012)

im confuse what to put..?? is it the date where i apply the spouse visa..as i rem that is a biometric?? do i need to put the date..

Yes, just put the info from when you applied spouse visa (date place etc)

....could anybody pls give mt the address of the VFS /Manila..
