hi there, i just started to fillin the form_an application for british ctizenship. in the form 1.5 surname and 1.6 names. where can i put/write my middle name?

so it means that i'll have to put my name and middle name in 1.6? sorry, i got another question in page1 1.1 about immigration and nationality directorate... i have not answer it yet because i don't know what to put. tnx
