Are you transfering though Heathrow or actually stopping in london. As if your in transit in london i don't think you have to have the medical check as you don't actually enter the country in Heathrow. Then Medical checks and passport control procedure, i guess would be up to Belfast or whichever Airport you end up in.

Maybe others can advise you more
